An application for a Certificate of Appropriateness (COA) shall be on a form provided
by the
Department and shall be submitted to the Department. A filing fee may be required pursuant to
administrative regulation. Such application shall include the following:
a. Owner’s name, street address
and legal description of the property involved.
b. A brief description of the present
improvements located on the property.
c. A detailed description of the exterior
modification proposed together with architectural
drawings or sketches, if applicable.
d. Name, address and telephone number
of the developer, contractor or architect, if
applicable.
e. Any other information as required
by the Director.
